FORMS FOR MEMBERS

Membership Form - All members must fill out this membership form and pay the required membership fee.

Policies and Procedures - All CCAT producers should be familiar with CCAT policies. These will be introduced during class orientation.

By-Laws - outlining all of CCAT organizational by-laws.

Statement of Compliance - All producers must sign this form acknowledging that they have read and understand CCAT's policies and producers.

Parental Permission Form - Any member under 18 must have this form filled out by his/her parent or guardian.

Cablecast Request Form - All producers must fill this out when submitting a program for cablecasting on CCAT.

Talent Release Form - Anyone that you interview for your program should fill this out. This is written authorization for use of a person's interview on your program. These will not be collected by CCAT but should be kept by the producer.

OTHER FORMS

Import Program Request Form - If you live in Carver and would like to sponsor a show that was produced in another town, fill out this form completely and send to CCAT. This must be filled out by both the sponsor and producer of the program.

Bulletin Board Form - This is to request an announcement from a non-profit organization be placed on the CCAT Channel 13 bulletin board. Announcements for Channel 14 must be sent to the Carver school administration for approval.

Program Copy Request Form - You can purchase many of the programs you see on CCAT Channels 13 and 14. DVD copies are $15 and VHS copies are $10. Please fill out this form, specifying the program that you want and send it to CCAT.
